The City of Normandy Park became a Passport Acceptance Facility in February of 2009, allowing us to assist with applications for a new US Passport Book or US Passport Card. The Acceptance Agent receives applications from the public and reviews them to ensure all necessary information and citizenship evidence is included. They administer the oath and witness the applicant’s signatures, then forward the applications to the Passport Service Agency (US Dept. of State) for issuance. The actual decision to issue or deny a passport book or card is made only by a Passport Services Agent, not the City of Normandy Park.

Contact the Normandy Park City Clerk at 206-248-8248 to make an appointment if you are eligible to submit a DS-11 application with required documentation. The form DS-82 is available at City Hall and is submitted by the applicant directly to the Department of State for processing.
